Dent Clinic is committed to fulfilling our corporate responsibilities in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA) introduced in January 1, 2004 and pertains to the collection, use or disclosure of personal information in the course of conducting business within Canada. Employees will collect and retain personal information in the normal course of conducting business on behalf of Dent Clinic only with the consent of the customer and/or other individuals, and providing that information is deemed necessary by the employee acting in Dent Clinic’s best interests. The information collected will be used or disclosed only for the purpose that was agreed to by the customer and/or other individuals. Dent Clinic is responsible for the personal information collected by its employees and systems and will retain that information only for as long as it is needed for the authorized purpose.
Definition
Personal information means any information about a customer and/or other individuals, with the exception of business contact information (business address, business telephone number, business email, business fax number – any information on a business card) and personal contact information that is listed in a public directory such as a telephone book.
Consent
Employees collecting personal information must ensure that the information is accurate and that the purpose for its collection is clearly identified and that the customer and/or other individuals have consented to such purpose. Exceptions would apply to information that Dent Clinic collects and that is required by law or law enforcement authorities.
Safeguards
Security safeguards appropriate for the sensitivity of the information will protect personal information. Should Dent Clinic be required to provide personal information to a third party, it is the responsibility of the Franchisee to ensure that standards are in place to protect that information.
Compliance
Dent Clinic will make available specific information about its Privacy Policy and practices relating to the collection of personal information to customers and/or other individuals. On request, Dent Clinic will inform customers and/or other individuals of the existence, use and disclosure of personal information that Dent Clinic maintains about them and give them access to that information. Customers and/or other individuals may challenge the accuracy and completeness of their personal information and request appropriate amendments.
Responsibilities
- It is the responsibility of every Dent Clinic employee to adhere to the key principles set out in the Privacy Policy
- Every Dent Clinic employee will determine if the collection of personal information on customers and/or other individuals for business purposes is necessary
- If Dent Clinic employees have determined that personal information they are collecting is necessary, they will take appropriate measures to protect that information so that it is kept confidential at all times and not shared with any unauthorized individuals
- Dent Clinic employees will ensure that they retain personal information only as long as it is necessary for the purpose for which that information has been collected and maintained
Should Dent Clinic employees determine at any time that they are in possession of personal information collected in the course of conducting business and that information is no longer necessary, they will delete or destroy that information immediately.
